If you are playing with the card, you are expected to have a way to make it clear which creatures are affected by The Cleanser Stone, and which number they got. A good way to do so is markers with numbers corresponding to the effect’s number on The Cleansing Stone.

The Cleanser Stone
{6}
Legendary Artifact
{2},{T}: Target creature gains on of the following (determine randomly):
1: Target creature is blue and gains “{U}{U},{T}: Counter target spell.”
2: Target creature is green and gains forestwalk and plainswalk.
3: Target creature is red and gains +3/+3.
4: Target creature is white and gains “{T}: Prevent up to 3 damage to target creature or player.”

The original text of the card does not involve the use of a die, but otherwise follows the template of Easter Egg and cards published by Wizards of the Coast such as Urza’s Science Fair Project which roll dice. It seems more consistent to incorporate die-rolling into this card as well.
