UnCommander Rulings
As the card and permanent or spell switch, they are not treated as entering or leaving any zones. Each inherits the state of the other. Both objects do each receive a new timestamp though.
Switching objects does not change the ownership of either.
If a token on the battlefield is swapped with a card in a hand, the token will cease to exist as a state-based action as normal.
