The Fallen Apart enters the battlefield with two arm counters and two leg counters on it.

Whenever damage is dealt to The Fallen Apart, remove an arm counter or a leg counter from it.

The Fallen Apart can’t attack if it has no leg counters on it and can’t block if it has no arm counters on it.

UnCommander Rulings

You do not have to mark the card itself to show that an arm or leg has been removed from The Fallen Apart (and it is highly unadvised).

The Fallen Apart’s original wording did not make use of counters to track the arms and legs. Mark Rosewater suggested the opposite, the use of markers to show which limbs had fallen off, in the Unhinged FAQ.

However, the use of markers to show the absence of something rather than its presence is confusing, and runs counter to how similar effects are presented on other cards, such as Sunset Pyramid.

Using counters to represent the presence of the limbs instead is more intuitive, although there are some gameplay differences, as it is subject to effects that deal with counters. However, many of these are flavourful, such as Hungry Hungry Heifer.
